To ensure competitiveness of our benefits package, wages and benefits are reviewed on a regular basis with data provided by means of national, regional and local surveys. In addition, the Human Resources Department and our external consultants receive ongoing updates in the areas of compensation and benefits administration. We believe that this ensures we are making changes to our package in a timely manner to be able to recruit and retain the industry’s top talent.

Health and vision coverage is available to all employees who average 30 or more hours a week. Employees are also eligible to enroll in Concord’s 401(K) plan, which offers a way for employees to save money for future retirement. A competitive matching program is in place, matching 25% of the first 4% an employee contributes to the fund.

Individuals at the management level are offered group medical, dental and vision insurance, in addition to the option to enroll in Concord’s 401(K) plan. Furthermore, these individuals will become eligible for various additional earned compensation at designated times during employment. This determination is made at the time an offer of employment is extended to a management candidate.  Each newly placed manager’s initial date of participation and eligibility in the Earned Compensation program is left to the discretion of the Area Director and/or Director of Operations.

Concord also has a rewarding vacation policy for both associates and managers.  Concord also offers sick and personal leave, as well as federal and state mandated leaves.

MANAGER BENEFITS

Medical Insurance:

Tier 1      

  • In Network = 90/10% - Major Medical
  • Out of Network = 70/30% - Major Medical                          
  • $15 Co-Pay for Office Visits
  • Deductible:  Single = $250; Family = $500
  • Maximum Out of Pocket:  Single = $750; Family = $1,500                  
  • RX Card - $10 Generic Mandatory
  • 25% Co-Pay ($20 min/$40 max) for Brand Name
  • Lifetime Maximum = $1 million                 
  • Vision is included

Tier 2

  • In Network = 80/20% - Major Medical
  • Out of Network – 60/40% - Major Medical
  • $40 Co-Pay for Office Visits
  • Deductible:  Single = $750; Family = $1,500
  • Maximum Out of Pocket: Single = $2,250; Family = $4,500
  • RX Card - $15 Generic Mandatory
  • 50% Co-Pay ($25 min/$75 max) for Brand Name
  • Lifetime Maximum = $1 million

Dental Insurance:

Tier 1

  • Deductible for Preventative Services - $0
  • Deductible for Basic/Major Services - $50
  • Preventative Services – 100% covered
  • Basic Services – 80% covered (6 month waiting period once coverage is effective)
  • Major Services – 50% covered (12 month waiting period once coverage is effective)
  • Orthodontic Services – 50% covered up to a lifetime maximum of $1,000
  • Orthodontia services initiated prior to effective date will not be covered
  • $1,000 maximum per person per benefit period

Tier 2

  • Deductible for Preventative Services - $0
  • Deductible for Basic/Major Services - $50
  • Preventative Services – 800% covered
  • Basic Services – 60% covered (6 month waiting period once coverage is effective)
  • Major Services – 40% covered (12 month waiting period once coverage is effective)
  • Orthodontic Services – no coverage
  • $750 maximum per person per benefit period – in network

Vision Insurance:

  • $15 Co-Pay for Exams, Frames and Lenses
  • Eye Exam are covered up to $60 every 12 months
  • Frames are covered up to $80 every 24 months
  • Single Vision Lenses are covered up to $50 every 12 months
  • Bifocal Lenses are covered up to $70 every 12 months
  • Trifocal Lenses are covered up to $90 every 12 months
  • Contact Lenses (medically necessary) are covered up to $200 every 12 months
  • Contact Lenses (elective) are covered up to $80 every 12 months

All staff participating in medical, dental or vision insurance may elect to defer premiums through the 125 plan, which allows premium deductions to be taken pre-tax, thus, increasing take-home.

Basic Life Insurance:

  • All employees electing medical coverage automatically receive $20,000 worth of term life insurance at no additional cost

Accidental Death and Dismemberment:

  • All employees electing medical coverage automatically receive $20,000 worth of coverage at no additional cost
  • Benefit is paid in the event of a loss of limb, vision, etc.

Supplemental Life Insurance:

  • Additional term life coverage (up to $500,000) is available for self and spouse
  • Term life coverage (up to $10,000) is available for dependent children
  • Premium depends on age and amount of coverage

Short Term Disability:

  • Replaces earnings at a rate of 66 2/3% up to a weekly maximum of $750
  • Waiting period of 14 days

Long Term Disability:

  • Replaces earnings at a rate of 66 2/3% up to a month maximum of $5,000
  • Waiting period of 90 days

Sick Pay:

  • Managers receive 4 days of sick pay per calendar year

Vacation:

  • Based on years of service
  • Eligible after 6 months
  • Renews annually at the first of the anniversary month
  • 1 to 4 years of service = 2 weeks vacation
  • 5 to 9 years of service = 3 weeks vacation
  • 10+ years of service = 4 weeks vacation

401K:

  • Eligible following 90 days of employment at the first of the next quarter
  • May contribute up to 25% of eligible income
  • Concord matches 25% of first 4% of eligible income
  • 100% vested after 3 years of service

Cellular Phone Discount:

  • Discounts available for Alltel and Sprint service
